Doc Onomical’s Terms & Conditions Of Subscription & Medical Service:

1. An annual subscription fee of $350 paid in advance. Term of service is one calendar year from the date of annual, in-full payment.

2. A per visit fee of $25 paid at the time of the visit.

3. Included services: Office visits, Primary Healthcare procedures, in-office lab tests, on-call coverage, Internet-accessed patient services.

4. Excluded services: injectable medications, vaccinations and tests (provided in-office via separate fee), laboratory fees, radiology and imaging fees, prescription medications, hospital care, and fees charged by referred-to healthcare providers.

5. Dr. Charles Berk is not responsible for any medical costs or charges incurred outside the scope of this agreement.

6. Early termination of subscription: if no medical services have been provided as of the date of requested early termination, the balance of the annual fee shall be refundable, prorated to the end of the current calendar month. If a Doc Onomical patient has made medical visits prior to early termination of the full term of service, the patient will be responsible for the cost of those visits. Doc O will calculate the billable cost based on Dr. Berk’s regular BHFP fee schedule on a fee-for-service basis. This billable cost shall not exceed the sum of $350, plus the visit charges incurred for those visits. 7. Due to rapid changes in the healthcare marketplace, Doc O reserves the right to change his subscription fees and/or his practice terms and conditions at any time without notice.
