Doc Onomical (aka Charles Berk, MD) seeks to promote and protect your health with Plan 350/25—his new, innovative and cost-effective Primary Healthcare service by annual subscription. Doc O has been successfully practicing Family Medicine in Brooklyn for many years. He is a Board Certified Family Doctor.
The Importance Of Primary Care:
Doc Onomical specializes in Primary Care. Primary Healthcare is the least expensive—BUT most important—part of keeping you well. Doc O believes in three Primary Care tenets: preventative medicine, proactive health maintenance and doctor-as-wellness-steward. Doc Onomical acts on these principles to keep you healthy and save you healthcare costs—in the broadest sense. Family physicians also specialize in wellness-services “gatekeeping”—as one of the recognized portals to other solutions within our complex U.S. healthcare system.

Our Plan’s compelling practice model enables us to deliver the real economy of Primary Care to you. Preventative healthcare is economical because it keeps you well. And, Primary Care treats you—early and effectively—when you’re not. In both the short and long term, this is the kind of healthcare that really saves you money.
Services, Included And Not:
Doc Onomical’s Plan 350/25 membership subscription supports Primary Care services. These include: office visits, Primary Healthcare procedures, in-office lab tests, on-call coverage and Internet-accessed patient services. These services are NOT included in the Plan: injectable medications, vaccinations and tests (provided in-office via separate fee), laboratory fees, radiology and imaging fees, prescription medications, hospital care, and fees charged by referred-to healthcare providers.
Payment Terms:
Plan 350/25’s title states the simple payment terms of Doc O’s groundbreaking program. To enlist in our Plan, we invite you to invest $350 in a yearly Doc Onomical membership subscription. Thereafter, you pay only $25 per office visit—comparable to an insurance-plan co-pay—regardless of how many office visits you make in the subsequent 12 months. Unless you decide to cancel your Doc O program participation, your subscription will be automatically renewed annually—via your credit card or PayPal account—on the date of your original payment.

Beyond Doc Onomical’s Plan 350/25 Primary Care, you may need complementary healthcare services. These will likely include prescriptions for appropriate medications, referrals to specialists, diagnostic imaging, lab tests and possible hospitalization. Doc O is also striving to provide affordable answers to questions—such as appropriate insurance plans—to buttress your overall healthcare regimen.
Doc O Net In The Making:
To provide you solutions for these healthcare contingencies and options, Doc Onomical is developing Doc O Net. This new network is comprised of a group of Dr. Berk-connected healthcare specialists and services who have decided to participate in the Net. When joining Doc O Net, healthcare providers pledge to offer our referred patients Doc O-like discounts and/or “Doc O-affordable” rates.
Healthcare A La Carte:
Today’s healthcare insurance usually covers primary care, specialists, lab tests, medicines and hospitalizations. Unless you’re very ill, you aren’t likely to need all of these services at the same time. In fact—with Doc Onomical—you may want to think of your healthcare options as a menu of low-cost choices. With Doc O’s flexible Doc O Net, you have the opportunity to order healthcare services as individually needed. Rather than paying for everything on the "menu," whether you require it or not.
Catastrophic Care Insurance:
Hospitalization is a major service that is outside Doc O Net’s subscription coverage. Unfortunately, today’s hospital care is extremely expensive. No ordinary person can pay for it without the help of some form of hospitalization insurance. Hence, Doc O STRONGLY encourages Plan 350/25 subscribers to purchase some form of catastrophic health insurance that covers hospitalization and emergency room services. Some kinds of hospitalization insurance plans are simply major medical plans with very high deductibles. Thus, they can be more affordable than regular healthcare insurance. Typically, monthly premiums for these "hi-deduct" plans run about $150 to $350/month for individuals. But—if you require intensive, extensive and/or expensive life-saving hospitalization or specialty care—these plans pay the bills, after you meet the deductible threshold.
Healthcare Continuity Through A Doctor Relationship:
With Plan 350/25, Doc Onomical also delivers healthcare continuity. Experts talk about the importance of an on-going doctor relationship. But—without some kind of “health insurance”—such relationships are hard for most people to maintain. Plan 350/25 will enable you to keep your healthcare “continuity via connection” going with your BHFP/Doc Onomical physician-relationship intact.
